Dear FBI,
Is it true you keep a file on every US citizen? Can I request mine? I'd just like to see what you have to say about me.
Also, anyway I can get a list of all the lists I am on?
Thanks,
Christopher L. Jorgensen

Note: I’m not going to reproduce the text of this form here, since the formatting would be impossible, but you’re welcome to click the image for a full sized scan.

My reaction:
I edited this letter slightly from what was actually sent, removing any overly personal identifying information I’d included. You don’t need to know that stuff. This is the first letter I have done that too without indicating what was removed. It’s none of your business.
I got back a form telling me that I didn’t provided enough information to have my request processed. Ouch! I wanted to fill out the known alias section and come up with tons of fake aliases. Like Christopher “Well Hung” Jorgensen, but then there’s the whole statement about perjury, and I wouldn’t want to go to jail because I’m not known to be well hung.
I like how this form came with a line highlighted for me. This line also says I can’t submit it, since it says my signature has to be legible. I’ve never had one of those. Even I can’t read my own handwriting.
